Towards a proactive approach for risk management and investigations (with Bjorn Feys)

Effective risk management is a key component of the Safety Management System. At Eurostar, we have been developing an approach that balances proactive risk assessment, and learning from incidents to assess the weaknesses in our systems. Using the Bowtie method to identify the controls and barriers in place to prevent against major incidents. Following an incident, the level of investigation assigned is based on the number of barriers that have failed, and the potential consequence, rather than the actual consequence.
